SMART Sat Subscriber Information Form and SIM Registration 
 
 

Smart Communications, Inc. (“SMART”, “We”, Us”) respects your fundamental right to privacy and we commit to take great care in safeguarding your personal data.

In compliance with the Data Privacy Act of the Philippines (Republic Act 10173) and with the SIM Registration Act (Republic Act No. 11934), we share with you the principles that govern how your personal data will be collected, processed, and shared, as you (a) create or update your account records through the Subscriber Information Form, and (b) register your SMART Sat Subscriber Identity Module (“SIM”) Card.

Why we collect your Personal Data

We collect and process the following Personal Data for the purposes specified below:

Personal Data Collected

Mobile Number/s of your SMART Sat SIM Card/s To register your SMART Sat SIM Card and Mobile Number, as required under the SIM Registration Act.
Integrated Circuit Card Identification Number or ICCID
*You may find the ICCID number at the back of your SIM card or in the SIM bed.
To validate that the SMART Sat SIM Card/s you are registering is/are in your possession and under your accountability.

*In the event that you are not able to indicate the matching ICCID number to the SMART Sat SIMs, we regret that we cannot register your SIMs and these will be subject to de-activation after the April 26, 2023 deadline. This is our mandatory security control measure. 

Complete Name
(First Name, Middle
Name, Last Name)

To register your SMART Sat SIM and Mobile Number, as required under the SIM Registration Act. Your name will be matched against the Government-Issued ID that you submit, in order to reasonably verify your identity.

In the event that the Complete Name you submit through the SIM Registration Form does not match the information we have in our account records, please expect a call-out from us to validate your information.

Birth Date
To register your SMART Sat SIM and Mobile Number, as required under the SIM Registration Act. Your Birth Date will be matched against the Government-Issued ID that you submit, in order to reasonably verify your identity.

In case that your Birth Date does not match the Birth Date in your Government-issued ID or if your Government ID does not contain a Birth Date, SMART will record the Birth Date you stated and attested to in the SIM Registration Form.

Your validated Birth Date will now be included as part of your information in our account records and will be used by us as a security measure to validate your identity whenever you initiate an after-sales request.
Nationality (for Foreign Nationals only)
If you are a foreign national, the following additional personal data will be collected from you, as required under the SIM Registration Act:

· A scanned copy of your Passport’s data page (with photo and signature). Your Name, Sex, Birthdate, and Nationality, as noted in your passport, will be used to validate the information you submit in the SIM Registration form.

 · Depending on whether you are visiting as a tourist or under another type of visa:

1. For Tourists
  • Proof of address in the Philippines
  • Return ticket showing the data and time of your departure from the Philippines
2. For Other Types of Visas
  • Proof of address in the Philippines
  • Alien Employment Permit issued by the Department of Labor and Employment
  • Alien Certificate of Registration Identification Card (ACRI-Card) issued by the Bureau of Immigration
  • School registration and ID (for students only)

Sex
To register your SMART Sat SIM and Mobile Number, as required under the SIM Registration Act.

Your Gender will now be included as part of your information in our account records.

Present or Official
Residence Address in the
Philippines

To register your SMART Sat SIM and Mobile Number as required under the SIM Registration Act.  In order to verify your identity, it will be checked against the Government-Issued ID you submit.

If your Residence Address does not match the address on your Government-issued ID, or if your Government ID does not contain an address, SMART will register the Residence Address you stated and attested to on the SIM Registration Form.

Depending on the type of Government-issued ID that you submit, you may inadvertently provide SMART with other personal data that are not required by RA 11934 (for example, the PhilSys ID includes information on your blood type and place of birth). SMART will not use said personal data as part of your SIM registration record.

Active Email Address
If we need to contact you regarding your SMART Sat subscription, we will use the email address you provide on the Subscription Information Form. Important Smart Sat service announcements may also be sent to your email address.  This is for SMART's use exclusively and will not be included in your SIM Registration Details.

Your submitted email address will be saved in our account records.

Alternative Contact
Numbers (i.e., Residence
and Office Landline
Number/s, and other
alternative Mobile
Number/s)

Because your SMART Sat devices are frequently utilized in unique circumstances and might not always be turned on, an Alternative Local Mobile Number will be collected to enable SMART to contact you through SMS or Calls.

Important SMART Sat service announcements may also be delivered to this additional Local Mobile Number.  This will not be a part of your SIM Registration Details and is used by SMART solely for its legitimate business purposes.

Selfie Photo
Your Selfie Photo will be used solely to match with the Photo on your Valid Government-issued ID. After we perform the visual check during documents verification, your selfie photo will be securely deleted from our email and systems immediately.


Authorized SMART personnel will collect, process, and verify the SMART Sat SIM Registration information that you submit through smartsatsimreg@smart.com.ph.

SMART shall retain your Personal Data throughout the duration of your SMART Sat service, and until ten (10) years after your service is terminated or your SIM deactivated.


Why is your personal data shared

Your Personal Data will be processed and stored in systems and databases provided by Salesforce.com (“Salesforce”).

We have engaged the professional services of Capgemini Philippines Corp. (“CapGemini”), to whom we have outsourced the technical support operations and enhancements of systems used in the SMART Sat SIM Registration process.

Salesforce and CapGemini (collectively, our “Service Providers”) may only process your Personal Data in connection with the above purposes for which they have been engaged.

SMART Sat services are joint solutions delivered by SMART and THURAYA TELECOMMUNICATIONS COMPANY PJSC (“THURAYA”, for brevity). THURAYA operates its network of satellites and provides the Satellite SIMs and Devices that are offered and marketed jointly by SMART and THURAYA to Subscribers.

SMART may share your information, including copies of subscriber identity documents when requested by THURAYA, but only when necessary for resolving issues of misuse or in the event THURAYA is obligated to comply with a valid and legal request of any regulatory or government authority.

When your Personal Data is shared with THURAYA and disclosed to our Service Providers, we ensure that it is done on a confidential basis, through secure channels, and in compliance with applicable privacy laws and regulations. Only personnel with a need-to-know will have access to your Personal Data.

How is your data protected

The integrity, confidentiality, and security of your Personal Data is important to us. This is why we strictly enforce this Privacy Notice and have implemented technical, organizational, and physical security measures that are designed to protect your information from unauthorized or fraudulent access, alteration, disclosure, misuse, and other unlawful activities. Your Personal Data is also protected from other natural and human dangers.

We likewise require all our Service Providers to implement the same security measures.
 

Your rights and choices

The Data Privacy Act gives you certain rights to your personal data. Subject to exceptions under the law, you are entitled to request:

  1. Access to the Personal Data we process about you;
  2. Rectification of your Personal Data if it is found to be outdated, inaccurate, or incomplete;
  3. Erasure of your Personal Data in cases where it is no longer needed to achieve the legitimate purpose of its processing;
  4. Restriction of processing of your Personal Data in limited circumstances;
  5. Portability of your Personal Data so that you may receive a copy thereof or that we transmit the same to another company on your behalf.
  6. Access to the Personal Data we process about you;
  7. Erasure of your Personal Data in cases where it is no longer needed to achieve the legitimate purpose of its processing;
  8. Restriction of processing of your Personal Data in limited circumstances;

Depending on the circumstances, such as when we process your Personal Data based on your consent, you may also have the right to object to the processing thereof. To exercise any of your data privacy rights, you may contact our Data Privacy Officer through:

If you believe that your data privacy rights have been violated in the processing of your SMART Sat SIM registration, we encourage you to contact the Company’s Data Privacy Officer to seek resolution of your complaint. Your rights hereunder are in addition to your rights under the Data Privacy Act of the Philippines. You also have the right to go directly to the National Privacy Commission or to make a claim against SMART before a competent court.
